A NOTE FROM THE SCHOOL NURSE...


Immunizations must be current -  Please provide us with an updated copy when your child receives immunizations.   This 
is district and state policy.

Medications- Medication  given 3 times daily or less should be given at home unless time sensitive.

1. A written request to administer medication form must be completed by the parent for all medications, both prescription as well as over the counter.  The medication form request must also be completed by a health care provider  for medications kept at school for more than 10 days.

2. Medications must be in the original container and properly labeled.  Parents requesting a dose different than the manufacturer instruction will require a health care provider’s  order.

Please keep your child home if vomiting (ill) or has a fever (temperature of 100 or greater).  This is district policy and 
helps to prevent the spread of illness to others.  Students can not return to school until they are fever free (temperature 
less than 100 degrees) for 24 hours without fever reducing medication.

Communicable Disease – Please report all communicable disease to the nurse’s office.  This information is useful in helping to 
prevent the spread if illness in the school.  Examples include strep, flu, pink eye, chickenpox, impetigo and scarlet fever.

Changes of Clothes- Please allow your child to carry a change of clothes in their backpack. This will prevent parents from 
making a trip to school.

Head Lice – Discuss with your child the importance of not sharing combs, hats and helmets to reduce the spread of head lice.  
Check your child’s hair at home on a regular basis.  Please notify the school nurse if your child has head lice.

Screenings – Vision and   hearing, screenings will be done in the fall  for  students in grades  Pre- K,  K ,  1, 3, and  5 .  Acanthosis  Nigricans will be done  for students in grades 1.3, and 5.   Screenings for students in other grades will be completed as requested, 

Clothes Closet – This service is two-fold.  If you need help with clothing for your child, please stop by the nurse’s office and 
request a referral form.  All requests are kept confidential. If you have gently used clothing that your child has outgrown 
please consider bringing the clothes to the school for donation.
